Living My Dream

Completing high school is a very important transition for all students. This is especially true for students with disabilities as they move into adult life. This manual is a student, family, and teacher resource for transition into publicly funded long-term care, integrated employment and achieving personal goals after high school.  

This manual was designed to be used as a resource for a wide audience. It can be read cover to cover or simply used as a resource for information on a specific topic such as Social Security or Integrated Employment.    

Find out about what options students have after high school? What supports are available? How do students plan for employment, independent living, and postsecondary education and training? These questions are part of the transition planning process.
